I recently tamed an Ancient Reptilian Flier and in my datapad, the icon for that creature is a cube with question marks on the sides. I don't know if this sort of thing has happened to anyone else, but it annoys me.

It has been that way since beta. There is nothing you can do other than /bug it.

Baz nitch (one breed if not all) have a camera icon instead of their own as well. Same problem.
